Coalition History

Since the 1990s, the community partners involved in the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) have been referred to by various names. Over the years, different priorities have emerged based on CHA data, addressing issues such as reducing mortality and morbidity from late prenatal care and decreasing youth bullying to promote better mental health. POWER Defiance County utilizes National Association of County and City Health Officials (NACCHO) strategic planning tool, Mobilizing for Action through Planning and Partnerships (MAPP), to guide the community health assessment and a community health improvement plan.

  • To gather data and insights about the health status, challenges, social determinants of health, and existing health resources within Defiance County.
  • Developing, monitoring and implementing the Defiance County Community Health Improvement Plan.
